It's odd to be posting after such a long absence - nearly a month and a half - but I felt like I should re-invest myself into this blog before I find myself completely out of the loop. I have thought about why I have lacked any means of motivation to post and I find it is two things: one, the weather has become an obstacle for it does not permit me to layer and thus I have found myself incessantly moping around and about my closet and as a result: two, my personal taste in style has been making a bit of a transition. Finding I can only generally wear one article of clothing at a time, I began acquiring a taste for collared shirts (don't ask me why because I don't know either. Miu Miu Spring 2010 could possibly be the catalyst but as much as I adore the collection I don't believe it had that much of an overwhelming influence in relevance to this 'style transition' I'm currently discussing. Also I feel like reading Virginia Woolf's The Waves had something to do with it but I haven't really discerned this yet and I'm beginning to digress so). Irrelevantly, I've found myself growing fonder of late 19th century and mid 20th century style as of late, a contrast in taste considering my favourite designers consistently challenge the constructs of what we know as fashion through their radical designs (McQueen and Chalayan primarily come to mind for a multitudinous amount of reasons). I'm not implying that I only admire post-modern designs, simply that they attracted my admiration more than collections that derived inspiration from the eras I currently admire with such a simplistic approach. Possibly now I'm just broadening my view rather than completely changing my style. I can't be entirely sure just yet. I'm writing this post with my mind in a rut regarding this topic so I might come off a bit misunderstood. Still trying to put some pieces together. However, I believe I should work on doing that in a different fashion than digressing in an oppressively constant manner on this here blog, don't you think?

So yeah. I've been relentlessly browsing vintage photographs of the past as of late, generally from the Tumblrs I follow or the Google LIFE archives. I've been posting the photographs consistently on my Tumblr so you can frolic over there if you want to see more but for now I'll simply post a few here for a taste.

I wore this outfit several weeks ago on a day where the weather finally allowed me to layer without dying if I stepped out into the blistering heat for more than five seconds.

(Pink blouse from Old Navy. All the skirts and the apron are thrifted except for one of the tulle layers which is a tutu from the kid's section as H&M five bucks baby. The socks are from the men's section at Urban Outfitters.)

Basically being in a style limbo plus lacking motivation for doing anything are the catalysts to my infrequent posting. I want to say I'll post again soon but I don't want to lie to you. I'll end this post here with something I wrote a while ago because I have started writing again on a regular basis just recently. Later days dudes.

it is true i attempt to suppress the feeling of indigence that perpetuates whenever memory is rekindled, mentioned in a whisper through the crevices of thought, working it’s way deep into the core of my mind. i blame my present tense by contemplating my recent past, and the actions committed regarding the circumstance. but this suppression is merely that - an attempt, susceptible to a demise like a human heart after death. that potentiality begins to decompose until it is nothing but a canvas marked with blank failures. as i realize this, the contrast of body and emotion begins to wither and they begin to form a link; they are brought together by collision in an objective sphere. my mind focuses on the fluctuation of resentment while my body battles its uncontrollable nature, and for an instance my being is at war with itself. but only for an instance, for one side eventually overpowers the other. in the end, my emotions become a lover’s figure under a silk slip, unsheathed at once, instantaneously.

Well, according to published reports 63 year old Camilla, Duchess of Cornwall, aka the wife of Prince Charles, has been in the British news lately for her apparent treatments of bee venom in lieu of Botox®.

Called an "organic facelift” for $85 you can undergo a masking procedure that "stings" the skin to stimulate production of bodily chemicals that will relax muscles and reduce wrinkles.

I think however that she still looks like time has taken a toll on her skin, I’ll stick with my Botox® and leave the hives to the royal wives.

Why is breast cancer reconstruction surgery with implants done in multiple stages? "Baby Steps"

From time to time you will get asked by breast cancer patients whether their reconstruction surgery can be done all in one stage at the time of mastectomy. The answer is you can, but there are a number of factors working against you for the best result, such that "baby steps" (planned sequential small procedures)

Typically, most implant reconstruction surgery involves placing a temporary implant called a "tissue expander" at the time of mastectomy that is later replaced by a permanent implant. As compared to a regular implant, a tissue expander is shaped different to maximize shape of the lower breast. It is decidedly more rigid and firm and then permanent devices, particular when silicone implants are later used. The advantage of such specialized devices is that they allow either 1) expansion of the skin by periodically adding fluid to them and 2) better resisting shrinkage of the skin following mastectomy.

Planned 2 stage surgery was popularized by Dr. Pat Maxwell (my mentor) and Dr. Scott Spears, and is well established as the most popular way to do breast reconstruction world wide. There has always been some interest in trying to skip the intermediate step, but doing it predictably is elusive. The big problem is tissue shrinkage of the skin, which as I mentioned is better resisted by the more rigid expander implant versus the softer permanent ones. The best candidates are those with smaller breasts who are having nipple-sparing procedures so that the native skin is 100% conserved. Even in that group, I find I'd be increasingly likely to go back and fat graft to camouflage the implant in a 2nd stage surgery. To my way of thinking, the benefit of single stage surgery just work enough to give up the benefit of the expander structural advantages.

Latisse (the eyelash drug) has been a raging clinical success

Quick thought of the day:

After dispensing Allergan's Latisse medication for enhancing eyelash growth for over a year now, I'm officially impressed. It is hand's down the single most reliable treatment we offer to patients, and I have not seen anyone who does not respond well with it. Part of it's popularity is also the relatively low price tag (~ $120-$130 for a 2 month supply).

The "off the record" advice I have for Latisse users is that I think you can actually use it less frequently then daily and maintain results. I advise patients that once they get to a good clinical result that they try every other or every third day for application. Rationing the medication like that can make a single box extend for 3-4 months instead of 2 without much diminishing results. A good cosmetic budgeting strategy in these times!

When is a medical record not a medical record? When the Obama adminstration get's PC with it


If the roll out of 21st century health care could have been more poorly handled, I'm not sure how. From a slow bleed over the spring involving an ill-conceived re-imagination of American health care delivery by the Democrats we are now presented with statements from President Obama's electronic medical record (EMR) czar that a medical record does not have to actually reflect what your medical history is.

Dr. David Blumenthal, the National Coordinator for Health Information Technology, said in an interview with CNS news (see here) that patients can choose to omit procedures such as abortions, positive HIV tests, or other perceived embarrassing information from their electronic health records (EHR).This is concerning in that a purported health record reporting a patient's comprehensive history could be edited so as to be politically correct. As a provider it would be important (for instance) to know that a patient had hepatitis or HIV before scheduling major elective procedures so as to protect oneself and operating room staff from unnecessary exposure or even advise patients to avoid some procedures altogether. Much as a physician has an informed consent with a patient, a provider must be aware of any and all material issues when delivering care.

Vampire Facelift


Yes, you read that right! Vampires are incredibly hot right now, and everyone wants a piece of the popularity it seems. ABC News is reporting there is a new product on the market called Selphyl that actually extracts the blood from your own body and is injected into wrinkles on the face and body. This product is being marketed in the same category as soft dermal fillers such as Juvederm and Restylane. They claim that the results are comparable to fillers and can last up to 15 months. In addition to great results the price range is in the ballpark of $1,100 to $1,500 per injection.

At first I wasn't too sure if I understood what this product was....I mean they are just taking blood and injecting it into wrinkled skin....um, isn't that just going to look like a bruise? Selphyl is prepared by drawing a patient's blood, separating the platelets from the red blood cells, blending the platelets with a fibrin mixture and injecting it to the area a patient wants to augment. It takes a few weeks to see the final results, so injectors are being told to overfill the area by 20% so the patient will have an idea what the final results will look like....the result "in the chair" will be a little larger than the end result. Of course with any new product there are some concerns. What to look for for well done breast augmentation surgery -The inframammary fold

This post is kind of an "inside baseball" topic about what surgeons look at when we judge our own or others work. One thing I fixate on more and more with cosmetic breast surgery is the position of the inframammary fold (IMF). The IMF (in layman's terms) is an anatomic landmark created by adherence of connective tissue to the chest wall. It defines the inferior border of the anatomic breast, and it's location makes it the most popular place for an incision to place breast implants via the "inframammary" approach.



One of the things I look for in someone I've operated on or whom comes in for revision surgery by another provider is where a prior inframammary scar is. If the scar is stable and in the position it was originally made in then I'm satisfied the surgical dissection was performed well. If the scar is now residing up on the skin of the lower breast, that suggests over release of the native IMF during prior surgery. Once violated, that anatomic border is hard to reliably recreate. Just a little extra attention during surgery can prevent a lot of issues down the road as it relates to this.
