Our First Spring Beauty

Our patient of the month for March is Teneia! After coming to the decision to have a breast augmentation, Teneia did her research and came to Dr. Walden for it. She wanted a natural look and she looks and feels great! She is so happy with her results, she spoke with LXNY about why she wanted to have the procedure done (video below). Sliced Capades

What's Real and What's Silver?


Singing, dancing and acting may not be her thing....so why did 4.7 million fans (including my coworkers and I) tune in this August for the premiere of this season's Keeping Up With The Kardashians? Perhaps we want to see the sense of reality or normalcy that we don't get to see on the airbrushed and perfectly positioned pictures we compare ourselves to. It is the same reason why we buy gossip magazines...our little guilty treasures. We want to see cellulite and stretch marks and bad hair days! Be honest: it makes you feel better about yourself. This is especially true when we see celebrities having injections done and plastic surgeries. Celebrity news shows often enlist the help of Dr Walden to decide who has had what done so the public can see what plastic surgeons see.

Now, that said, the Kardashian show has as many fans as it does because we feel they are being real with us. Kourtney has had breast implants and she openly talks about it. So why does Kim deny having any breast-related surgeries and more importantly are we being cheated? In this W spread, everything seems to be out in the open and quite shiny and silver. To those who watch the show, we know that Kim turned down a nude shot for Playboy. So why did she then pose nude for W? Maybe it is because the silver paint is hiding signs of breast implants. To the untrained eye this might be true but take a closer look!  In the picture, the shading of the silver paint as it reflects off the flashes of the camera shows that Kim could be dealing with change in breast shape due to her implants. It's called capsular contracture and it occurs when excessive scar tissue forms around the implant and tightening occurs. Large, heavy implants placed in the subglandular position (instead of under the muscle of the breastbone) can more frequently end up looking tight and misshape the breast (causing tissue stretch and indentation in the skin). Could this be what we are seeing this on the inner aspect of the left breast more than the right in the W photo?

So, Kim, just tell us all! We promise to still like you and most of us will like you more if you come clean. We know that you were blessed with a derriere of extreme proportions. We know that it must have been hard being so un-proportioned and you wanted to be more balanced. We all want the same thing and we are some even have plastic surgeons to help us out. We are not born perfect...especially since we all have a different idea of what perfection is. The important thing is that we can be happy and we can get the look we want with a little help from our friends with steady hands and skills with the scalpel. What do you think?

What to look for for well done breast augmentation surgery -The inframammary fold

This post is kind of an "inside baseball" topic about what surgeons look at when we judge our own or others work. One thing I fixate on more and more with cosmetic breast surgery is the position of the inframammary fold (IMF). The IMF (in layman's terms) is an anatomic landmark created by adherence of connective tissue to the chest wall. It defines the inferior border of the anatomic breast, and it's location makes it the most popular place for an incision to place breast implants via the "inframammary" approach.



One of the things I look for in someone I've operated on or whom comes in for revision surgery by another provider is where a prior inframammary scar is. If the scar is stable and in the position it was originally made in then I'm satisfied the surgical dissection was performed well. If the scar is now residing up on the skin of the lower breast, that suggests over release of the native IMF during prior surgery. Once violated, that anatomic border is hard to reliably recreate. Just a little extra attention during surgery can prevent a lot of issues down the road as it relates to this.

Teresa Giudice Speaks Out About Her Breast Augmentation

Teresa Giudice, from Bravo’s hit series the Real Housewives of New Jersey, is known for being an outgoing, table flipping Italian princess. She also has been getting media attention for her onscreen footage of her Breast Augmentation procedure that she underwent.

The 37-year old told Steppin’ Out magazine that she is “much more confident with my body now,” was an A-cup, and “ after [she] breast fed, they looked different. [She] looked at them and went, Ewwwwww. They were deflated. [She] was just a big nipple.”

Giudice also said that she “never liked how [she] looked in bathing suits. [She] never wanted to put lingerie on.” And she feels that “you need boobs for lingerie.”

Having been married for nine years before the surgery, Giudice says that her husband did not care about her small breasts and he loved her the way she was, but that she got the procedure done for her, is happy and feels “so much sexier.”

And avid watchers of the show will know that the real gems of New Jersey are her spoiled, sassy, but adorably cute, four young daughters, Gia, 9, Gabriella, 5, Milania, 4, and Adriana, less than a year old. Giudice says that if her daughters want the procedure when they are older, she will fully support them.

Now that her procedure has settled she is showing off her new breast, in typical Teresa fashion: loud and proud.

LXNY Segment Boobs vs. No Boobs

LXNY produced a GREAT segment yesterday about the dilemma to get breast augmentation or not. This is always such a hot topic no matter what is going on in the media. As it would have it people like Kate Hudson and Heidi Montag gave these ladies PLENTY to discuss. In addition to experts from magazines like In Touch, was our own Teneia Wooten. Ms. Wooten is a recent patient of Dr. Walden who underwent breast augmentation. Teneia is such a excellent example of the kind of women that make up the majority of patients that undergo breast augmentation. I think with Hollywood stars like Heidi Montag and Pamela Anderson splashed all over the tabloids, we as a society associate breast augmentation with those type of results. To be honest woman like Teneia who have a natural and beautiful result are more the norm.

Many issues are discussed such as breast size, body dysmorphic disorder, and other self confidence issues. We all thought Teneia's answers were spot on! She also brings up a good point that breast augmentation is a big decision and a major surgery. It is incredibly important to always do your research and make sure you go to a board certified plastic surgeon when thinking of going under the knife. Especially when these days we have easy access the Internet which can be such a great research tool!

Are your breast implants under warranty?

The New York Times had a story last week (see here) on how expensive orthopedic implants for knees and hips were to replace when they failed. As the cost of the devices themselves (without hospital or physician charges) can run north of $15,000, it can be more then $50K on the price tag when these patients require re-operation for premature failure. Highlighted in the story was the fact that the manufacturers did not expressly provide a warranty for replacement costs of their implants.

Much like these orthopedic devices, patients with breast implants can expect their devices to have to be replaced at some point in their life, either thru device failure or for aesthetic revision issues as their body changes. In contradistinction to the orthopedic companies, both Allergan and Mentor Corp. (whom combined sell 99% of all breast implants in the United States) have offered lifetime replacements on their failed devices for several years now. Allergan in particular has been admirable, as the parent company that makes the implants has been sold twice from it's roots as McGhan medical (later Inamed Corp.). As both Allergan and Johnson & Johnson (owner of Mentor Corp. since 2009) are huge multinationals, it would seem patients with these implants should have a good deal of security of their devices fail for replacement of their implants.

These implant companies do not however cover all other costs associated with the replacement of the devices, but have come up with a fairly generous standard program matched by both Mentor & Allergan

  • 10 years of guaranteed financial assistance
  • Up to $1200 in out-of-pocket expenses for surgical fees, operating room and anesthesia expenses not covered by insurance
  • Silicone filled and saline filled breast implants are both covered
  • Lifetime product replacement
  • Automatic enrollment at the time of your original surgery
Now as a breast implant is relative expensive to design and test clinically, but inexpensive to manufacture by the unit, it's easy to see how these companies can absorb the cost. I don't know exactly their margin per device, but I think it's $600-700+ per silicone implant they sell.
